M-23 had its western terminus at the Indiana/Michigan state line near Elkhart, Indiana

 , crossing the state line in a north–south direction along a route later known as M-205

. M-23 turned abruptly eastward about two miles north of the state line and followed a route that with some later re-alignments would become US 112 from 1926 until 1961 and in turn US 12.
