The 2011–12 First Macedonian Football League will be the 20th season of the First Macedonian Football League, the highest football league of Macedonia
. It will begin on 31 July 2010 and is scheduled to end at an yet unknown date. Shkëndija Tetovo are the defending champions after winning their first Macedonian championship at the end of the 2010–11 season.

Because of a 1–4 defeat in their play-off, FK Skopje immediately returned to the Second League after just one season at the highest level of Macedonian football. The other relegation/promotion play-off featured tenth-placed Napredok Kičevo
and third-placed Second League team GFK Tikveš; Napredok successfully retained their spot in the league after beating the Second League team by 2–0.
The relegated teams will be replaced by 2010–11 Second League champions 11 Oktomvri
from Prilep
and runners-up FK Ohrid
