"The Heart of the Matter'" is the 4th episode of the fourth season of the ABC
series, Grey's Anatomy
. The episode was written by Allan Heinberg and was directed by Randy Zisk. The episode originally aired on October 18th, 2007.
This episode deals with George having told Callie that he slept with Izzie. Although Callie forgives George because he had had a hard year, she wants to talk to Izzie about it, but Izzie and the rest of the medical staff are led to believe that a fight in the cafeteria is going to occur between the two. It ends with Izzie trying to apologize to Callie. Izzie also tells Alex, and he is astounded.
Other things that happen in this episode: Meredith has to clean up after Norman when he mistakenly tells a patient she is going to die because he misread the charts, and Cristina has to reconsider her attitude towards interns and also take charge of the potential flirting between Derek and Lexie. The Chief has to deal with his niece (Camille) being admitted to hospital again for more surgery and her wanting to go home.
The episode ends with Callie deciding and realizing that she doesn't forgive George in the end, going to bed alone that night, Derek professing his love and his wish for a beautiful, long lifetime with Meredith but also saying that he'll wait until she's ready, and Lexie thanking Cristina for saving her in the surgery earlier in the day (but Cristina still calls her "3").

A face familiar to one of the year-twos shows up and dredges up issues that were never really dealt with. A teen football player needs counseling because of his father and surgery for an injury he sustained while playing the game. The girl who was the reason for the prom at Seattle Grace finds out her cancer is back and she brings her aunt Adele with her.This episode deals with George having told Callie that he slept with Izzie.
