The album's cover art, which features the band's logo along with a white and pink design which resembles a keyhole, or a turntable, or a ladies restroom sign, has been noted by some to be unusual. Some considered this unexpected after the band's previous covers, most of which were drawn by Wes Borland

 himself. When asked about the album art, Durst stated it was his idea and it was done deliberately: "I wanted it to stand out on the shelf and I did not want it to look like every other metal band record." He also refused to explain the meaning of the cover, saying it was open to interpretation.

DVD, compiling most of the band's music videos, was released on the same day, separately from the album. The videos for "Sour", "Take a Look Around", "The Truth" (which was not an official music video), the second and third version of "Behind Blue Eyes", and the video for "Home Sweet Home/Bittersweet Symphony" were all omitted from the DVD, to the dismay of many fans.

Many of the tracks are the radio edits, as opposed to the album versions, for example Counterfeit is shorter, and the interludes following many of the tracks on the original albums are taken out.
