The U.S. Agency for Healthcare Research and Quality
(AHRQ) created the Health Care Innovations Exchange to speed the implementation of new and better ways of delivering health care and reducing health disparities
nationwide. Launched in 2008, http://www.ahrq.gov/news/press/pr2008/innovationspr.htm>, the AHRQ Health Care Innovations Exchange offers busy health care professionals and researchers a range of opportunities for sharing, learning about, and ultimately adopting evidence-based innovations and tools suitable for a range of health care settings and patient populations.

The AHRQ Health Care Innovations Exchange also offers opportunities for participants to learn from and share with innovators and other experts through interactive webinars and a web-based discussion series. The webinars feature cutting edge issues in health care, such as medical information exchanges, using data for quality improvement, and learning from attempts. The web-based discussion series "Chats on Change" features innovative health care topics such as the role of clinical nurse leaders as quality champions and effective provider-patient email.
In addition, the AHRQ Health Care Innovations Exchange hosts the Coordination of Community Care Learning Network (CCCLN)http://www.innovations.ahrq.gov/learn_network/about.aspx (by invitation only) to bring communities together to adopt care coordination models that can reduce health disparities
. The learning network currently uses the Pathways Model to connect at-risk populations to needed care.
Two groups of experts ensure that the information and services on the AHRQ Health Care Innovations Exchange reliably meets its users' needs.
