I work in QC department. Part of the job of QC team is to monitor the NCR (Non-Conforming Report).
Every year QC team is struggling to achieve company's objective.
But unfortunately we failed. Yr'2011 total NCR cost is around 60K ringgit malaysia.
Most of the mistakes are repeated and majority root causes were "careless".
Management wanted to penalize all the the mistake maker.
The penalization can be "show cause letter" given or PIC have to bear the cost upon the mistake they made.
Why company have to do so strictly, because the issue occur since long time ago.
Ladies & gentlemen, do you thing penalization is the best way to solve the issue?
If no, could you please provide some "SMART" (Specify, Measureable, Achieveable, Responsible, Timeline)ideas as a guideline to come out a better improvement plan. We look forward to all of your valuable ideas. Thank you very much.

Sounds like you have management issues higher up. If no one is taking responsibility for the quality control, Id find another job or document everything & give it to the big boss & let him deal with it.
