I know for sure that part of the space craft started falling off the ship. What I don't know is what caused it? Was it the waether? Was it that the space craft could no take that kind of usage?

A piece of foam insulation broke off the Space Shuttle external fuel tank during launch. As a result, the shuttle overheated and disintegrated when it re-entered the Earth's atmoshphere several days later.
