I need to obtain a birth certificate and a death certificate for a family member who was born (1926) in and died (2004) in Santiago de los Cabellos. I called the Dominican Embassy for assistance, but after a month, they have been unsuccessful in helping me. It would appear that I need to have a copy of the documents in order to provide a folio number. I DO have a photo copy of the death certificate, but it was crooked on the copier, and the folio number is not there, and the last digit of the birth year is cut off.

Can anyone help me?
