Our first experience with transdouglas endoscopic surgery was at the Mount Sinai Hospital of Queens in 1998; at that time, we named that surgery Culdolaparoscopy. Hybrid Culdolaparoscopy is a MANOS that combines operative culdoscopy with minilaparoscopy (www.culdoscopy.com). Hybrid Culdolaparoscopy uses a sealed vaginal port that avoids the problem of losing the pneumoperitoneum; the port, which is placed under minilaparoscopic surveillance, is used for insufflation, visualization (either with a laparoscope or a gastroscope), to place large instruments, and to place an endoscopic bag. The endoscopic bag can be easily introduced and extracted via the vaginal port, and, when needed, the pneumoperitoneum reinstated.
